2. Kufanyiwa Uchunguzi wa Afya:
   • Hakikisha fomu imejazwa na daktari aliyesajiliwa rasmi.
3. Kuwasilisha Fomu:
   • Wasilisha fomu iliyojazwa wakati wa kuripoti chuoni.
